Please can you arrive 10 minutes before your appointment time and ensure you bring the following items when attending:
Photographic means of identification. Acceptable identification includes a passport, Driving Licence that includes a photograph) or Sentinel card. National Insurance Number
Details of Medication. You will need to provide the names and dose of all medication taken in the last 14 days whether prescription or over the counter medication.
You are encouraged to bring a copy of your prescription or the medication itself to ensure an accurate declaration.
Spectacles or contact lenses - if worn. Hearing Aids – if worn.
*If you are attending for medical assessment, you are advised to avoid exposure to loud noise and noisy situations in the 12 hours prior to testing.
†If you are attending for drug and alcohol screening, you are advised not to smoke, use mouthwash or consume either food or drink in the 15 minutes prior to your appointment
